About the Role

FOX Rehabilitation is growing in Three Rivers, Michigan, and they need a licensed Physical Therapist ready to treat older adults right where they live. That could be a private home, an assisted living facility, memory care, or independent living — wherever the patient calls home. FOX built its whole model, called Geriatric House Calls, around exactly this kind of one-on-one care.

You won’t be stuck following someone else’s calendar here. FOX lets you build and manage your own caseload, so you treat patients when it actually works for both of you. On top of in-person visits, you can fold telehealth sessions into your week too, which means more flexibility for you and better continuity of care for the people you’re treating.

This is hands-on work, so come ready for it physically. Expect to lift 25 to 50 pounds occasionally, 10 to 25 pounds frequently, and up to 10 pounds pretty much constantly through the day. You’ll also help with patient transfers, bed mobility, standing and walking for stretches at a time, climbing stairs, and driving between homes and senior living communities as part of the job.

Primary Responsibilities

  Treat older adults one-on-one across homes, assisted living, independent living, and memory care

  Evaluate patients and build individualized, evidence-based treatment plans

  Document care inside FOX’s EMR system to meet regulatory standards

  Work alongside PTs, OTs, SLPs, and Fitness Specialists as one team

  Coach patients, caregivers, and senior living staff on safety and aging in place

  Add telehealth visits to your schedule when it makes sense for the patient

Qualifications

  Valid Physical Therapy license (or eligible to obtain) for Michigan
• ✓
  Degree from an accredited Physical Therapy program
• ✓
  Basic computer and EMR proficiency

  Current CPR certification
